Resumen

La enfermedad de Fabry es una enfermedad de depósito lisosomal ligada al cromosoma X que produce deficiencia de galactosidasa alfa A. Es una enfermedad difícil de diagnosticar debido a la heterogeneidad de sus manifestaciones clínicas, donde predominan manifestaciones clásicas como angioqueratomas, córnea verticillata, proteinuria, enfermedad renal crónica que requiere terapia sustitutiva, hipertrofia ventricular izquierda, arritmias y enfermedad cerebrovascular. Se describen principalmente dos fenotipos, uno clásico que es muy poco frecuente, más severo y se manifiesta a edades tempranas, y un fenotipo tardío que se manifiesta a mayor edad y tiene como principales órganos blanco corazón, riñones y sistema nervioso. Su fisiopatología es compleja, relacionada al acúmulo de metabolitos derivados de glucoesfingolípidos, así como la disfunción endotelial y estado protrombótico que estos generan a nivel sistémico. El diagnóstico se realiza al evidenciar una función enzimática disminuida, posterior a lo cual se realiza un estudio genético confirmatorio. Es de gran relevancia el diagnóstico oportuno, ya que existe terapia de sustitución enzimática eficaz, la cual potencialmente modifica la historia natural de la enfermedad e incrementa la sobrevida de los pacientes.
