Adnexal torsion in pregnancy: a literature review

Abstract
Adnexal torsion is an uncommon pathology that occurs at any age, being more common during reproductive years. Pregnancy is considered a risk factor, especially in patients with history of reproductive assistive technics and ovarian pathology. Clinically, is characterized by an acute inferior abdominal pain and nonspecific symptomatology, easily confused with other gynecologic, urologic and gastrointestinal pathologies. The use of imagenologic studies is vital to make the diagnosis, and the first line study is the ultrasound; the magnetic resonance imaging is a reliable alternative to diagnose adnexal torsion when the result of the ultrasound is undetermined. Once the diagnosis is made, immediate intervention is necessary in order to preserve the viability of the ovary. Laparoscopic surgical treatment has proven to be safe at any trimester of pregnancy, and is considered superior than laparotomy in terms of surgical results, shorter hospitalization and better materno-foetal outcomes. The prognosis is good for those women treated early and has a low rate of recurrence, usually related with concomitant adnexal pathology.
This review aims to provide the basic knowledge for an optimal approach and diagnosis to avoid complications related to adnexal torsion.
